Centerville Clinics Sliding Fee Program
Centerville Clinics accepts all patients regardless of their insurance or financial status. We are a federally funded health care program which in turn allows us to offer a wide range of services to patients through the sliding-fee scale. We offer the sliding-fee scale to all income eligible uninsured or under-insured patients based on annual household income for most of our services. Income guidelines and acceptable verification for the sliding-fee scale are as listed below. How do I qualify?
If you wish to qualify for the Sliding Fee you MUST show proof of gross annual income for all immediate family members living in your household. Gross income is ALL income from ALL sources before taxes. Applicants should provide a copy of either three consecutive pay stubs, or the previous years’ income tax return or W-2 form.
Acceptable Income Verification
• Recent Federal tax return complete
• (3) Current paystubs and W-2 or 1099
• Proof of any form(s) of income not reportable on IRS tax return

Open Enrollment for ACA plans through Pennie 11/1/2023 – 1/19/2024
Centerville Clinics, Inc. has Enrollment Assisters that can assist with applications for coverage through Pennie, Pennsylvania’s ACA marketplace. Assisters help with applications and discuss differences between all available plans and answer all questions regarding Pennie plans.
Assisters also assist with initial applications and/or renewal of State programs MAWD, MA, CHIP, PH-95, and Medicaid Expansion.
Private one-on-one appointments are available at 11 offices in the tri-county service area. Individual will choose office that is most convenient and all assistance is free of charge.

Prescription Assistance Program
Centerville Clinics’ Prescription Assistance program assists our uninsured and underinsured patients to obtain their prescribed medications by providing prescriptions at significantly reduced prices. Centerville Clinics Inc. (CCI) participates in the Federal 340B program and other pharmaceutical manufactures programs.
