NCQA Patient-Centered Medical Home Recognition

We are excited to announce that the National Committee For Quality Assurance (NCQA) has granted all 12 offices of Centerville Clinics Patient Centered Medical Home recognition. NCQA’s Patient-Centered Medical Home (PCMH) Recognition program is the most widely adopted PCMH evaluation program in the country. More than 10,000 practices (with 50,000+ clinicians) are recognized by NCQA.

The patient-centered medical home is a model of care that puts patients at the forefront of care. PCMH builds better relationships between patients and their clinical care teams. Research shows that PCMH’s improve quality, the patient experience and increase staff satisfaction while reducing health care costs. Practices that earn recognition have made a commitment to continuous quality improvement and a patient-centered approach to care.

This recognition is also significant in that an independent, third party has certified that Centerville is providing high quality, compassionate care to our patients.

Healthcare providers who have adopted the PCMH model have proven to:
1. Reduce Fragmentation and Improve Quality
2. Improve Staff Satisfaction
3. Improve Patient Experience
4. Better Manage Chronic Conditions
5. Align With State/Federal Initiatives
6. Improve Patient-Centered Access
7. Align With Payers
8. Lower Health Care Costs

Centerville Clinics chose to be part of the ever-changing healthcare system and to continually improve patient care. PCMH aligns with this mission, and it is more than just a certification. It is an entire transformational process touching on all aspects of staff and patient well-being. By practicing as a Patient Centered Medical Home, it will help Centerville Clinics streamline processes, reduce redundancies, and increase quality of patient care. We believe that this will ensure organization throughout all offices and departments.

We are honored and excited to now be certified as a Patient Centered Medical Home.
